YOUR ANNUAL HEALTH RISK APPRAISAL INCLUDES:
STEP ONE - WELLSCREEN: clinical tests of your cholesterol, blood pressure and weight through Occupational Health, ViQuest or your personal provider.
STEP TWO - PERSONAL WELLNESS PROFILE: questions about your nutrition, exercise and other health behaviors. Please have your WellScreen results in hand so you can enter them into the profile also. When you are done, read your report to learn more about your health risks and tips on how you can improve.
